# Review a failure and authorise a full retest

Inspect the original evidence, then retain a reason for a complete new inspection.

Supervisor access is required. The fixture contains a completed, nonconforming inspection recorded by a separate operator. An independent author and reviewer approved its requirements.

## Review a failure and authorise a full retest

1. **Open the prepared inspection record**

   Open the prepared inspection record.

   You are signed in as Sarah with supervisor access only. A different operator has completed this fictional inspection, recording 10.1001 millimetres against a 10.10 upper limit.

2. **Check the original unit details**

   Check the original unit details.

3. **Read the retained observations**

   Read the retained observations.

   Read every required observation, including the failed reading. Completing the checks did not release the unit.

4. **Review the full retest requirements**

   Review the full retest requirements.

   Authorise a full retest only when the required follow-up work and local procedure support it. All applicable checks must be repeated.

5. **Explain the retest decision**

   Explain the retest decision.

   Record why a fresh inspection is justified. This keeps the original failed attempt.

6. **Authorise the full retest**

   Authorise the full retest.

   The next attempt is now permitted. No fresh measurement has been made by this action, and supervisor release will still be required.

The original failure and hold remain. Rework, retesting, hold resolution and release are separate steps; authorising a retest does not release the physical unit.
